A case of carcinoma rectum T3 disease on CT/ MRI;radiologically not invading any nearby other organ; with gross radiological evidence of external iliac node. what will be the stage in view of the external iliac radiologically present. Otherwise also, what will be the stage if there is evidence of significant external iliac node on radiological studies.

Hi Abhinav,
As per the AJCC 2002, the following are the regional lymph nodes for rectum:

1. Perirectal,
2. Sigmoid mesenteric,
3. Inferior mesenteric,
4. Lateral sacral
5. Presacral,
6. Internal iliac,
7. Sacral promontory (Gerota’s),
8. Superior rectal (hemorrhoidal),
9. Middle rectal (hemorrhoidal),
10. Inferior rectal (hemorrhoidal)

As you can see external iliac nodes will be technically considered non regional and stage designation should be M1 (see Fig 12.10 of the AJCC handbook for a reference example).
Radiological presence may be misleading so a FNAC will be advisable for confirmation of the diagnosis.
